Time trends of physical activity and television viewing time in Brazil: 2006-2012
Background: Despite recent advances in surveillance of physical activity, data on time trends of physical activity in low and middle-income countries are lacking. This study describes time trends in physical activity and television viewing between 2006 and 2012 among Brazilian adults. Methods: Data from 371,271 adult participants (18 + years) in the Surveillance System for Risk and Protective Factors for Chronic Illnesses using Telephone Survey (VIGITEL) were analysed. Time trends in leisure-time physical activity (≥ 5 days/wk; ≥ 30 min/day), transportation physical activity (using bicycle or walking for ≥ 30 minutes per day as a means of transportation to/from work) and proportion of participants spending more than three hours per day watching television were analysed. Annual changes according to sex, age and years of schooling were calculated. Results: There was an increase in leisure-time physical activity from 12.8% in 2006 to 14.9% in 2012 (annual increase of 1.9%; p < 0.001). This increase was more marked in younger participants and those with high-school education. Transportation physical activity decreased 12.9% per year (p < 0.001) from 2006 to 2008 and 5.8% per year from 2009 to 2012 (p < 0.001). The annual decline in television viewing time was 5% (p < 0.001) between 2006 and 2009 and 2% (p = 0.16) between 2010 and 2012. Conclusion: National survey data from Brazil indicate that leisure-time physical activity appears to be increasing, while television viewing time appears to be decreasing in recent years. However, transportation physical activity has been declining. These data are important for informing national public health policies
Clustering of risk factors for chronic diseases among adolescents from Southern Brazil
AbstractObjectiveTo investigate the clustering of risk behaviors for chronic non-communicable diseases and their associated factors among adolescents from Southern Brazil.MethodsIn 2008, a survey was conducted with 3990 adolescents aged 14–15years (mean: 14.3; SD: 0.6) from the 1993 Pelotas Birth Cohort Study. Clustering was determined by comparing observed (O) and expected (E) prevalence of all possible combinations of the four risk factors investigated (smoking, alcohol intake, low fruit intake, and physical inactivity). We carried out Poisson regression to evaluate the effect of individual characteristics on the presence of at least three risk behaviors.ResultsAll risk factors tended to cluster together (O/E prevalence=3.0), especially smoking and alcohol intake (odds ratio to present on behavior in the presence of other >5.0). Approximately 15% of adolescents displayed three or more risk behaviors. Females (adjusted OR=1.55), people 15years and older (OR=1.47), with black skin color (OR=1.23), and of low socioeconomic level (OR=1.29) were more likely to display three or more risk factors.ConclusionThese findings suggest that lifestyle-related risk factors tend to cluster among adolescents. Identifying subgroups at greater risk of simultaneously engaging in multiple risk behaviors may aid in the planning of preventive strategies
Validity and reliability of self-report instruments for measuring physical activity in adolescents: a systematic review
Realizou-se revisão sistemática de estudos de reprodutibilidade e validade de instrumentos de medida da atividade fÃsica do tipo self-report - medidas subjetivas, em adolescentes (10-19 anos). Buscas foram realizadas em bases de dados (MEDLINE, PsycInfo, SportsDiscus, Scopus, Web of Science, SciELO, Lilacs) e nas referências dos artigos localizados. Sessenta e seis estudos atenderam aos critérios de inclusão. A maioria deles foi realizada em paÃses da América do Norte, apenas cinco no Brasil. Identificaram-se 52 instrumentos diferentes: 42 questionários, 6 diários e 4 entrevistas. A reprodutibilidade "teste-reteste" variou de 0,20 a 0,98; a maioria (28/50) dos coeficientes apresentou valores < 0,70. Os coeficientes de validade apresentaram ampla variação (-0,13 a 0,88), sendo a maioria deles (64/84) < 0,50. Apenas três instrumentos apresentaram correlações > 0,70. Diversos instrumentos foram testados em adolescentes, especialmente questionários. Em geral, tais instrumentos demonstraram melhor reprodutibilidade "teste-reteste" do que validade.This was a systematic review of studies on the reliability and validity of self-report instruments for measuring physical activity, or subjective measurements, in adolescents (10-18 years). Searches were conducted in databases (MEDLINE, PsycInfo, SportsDiscus, Scopus, Web of Science, SciELO, Lilacs) and in the references of the retrieved articles. Sixty-six studies met the inclusion criteria. The majority were from North America, with only 5 from Brazil. Fifty-two different instruments were identified: 42 questionnaires, 6 diaries or logs, and 4 interviews. "Test-retest" reliability varied from 0.20 to 0.98; the majority (28/50) of the coefficients showed values < 0.70. Validity coefficients showed wide variation (-0.13 to 0.88), with the majority (64/84) < 0.50. Only 3 instruments displayed correlations > 0.70. Various instruments were tested in adolescents, especially questionnaires. A Descriptive Review of the Methodologies Used in Household Surveys on Medicine Utilization
Background: Studies carried out in the community enable researchers to understand access to medicines, affordability, and barriers to use from the consumer's point of view, and may stimulate the development of adequate medicines policies. The aim of the present article was to describe methodological and analytical aspects of quantitative studies on medicine utilization carried out at the household level. Methods: Systematic review of original papers with data collected in studies in which the household was a sampling unit, published between 1995 and 2008. The electronic review was carried out in Medline/Pubmed, Scielo and Lilacs. The reference lists of the papers identified were examined, as well as other publications by their authors. Studies on the utilization of specific pharmacological groups, or those including only respondents with a given disease were excluded. Results: Out of 4852 papers initially identified in the literature search, 61 fulfilled our inclusion criteria. Most studies were carried out in Europe and North America and used a cross-sectional approach. More than 80% used face-to-face interviews for data collection, and the most frequently used recall period for assessing medicine utilization was 14–15 days. In 59% of the studies, interviewers were trained to request the packaging of the medicines reported by the subjects; medical prescriptions were requested less frequently (15% of the studies). Conclusion: These data will be useful for updating researchers on what methods their peers are currently using. Such information may help overcome challenges in the planning and analyses of future studies. Moreover, this publication may contribute to the improvement of the quality of medicine use data obtained in household surveys
Prevalência de atividade fÃsica em adolescentes e fatores associados
OBJECTIVE: To estimate the prevalence and identify correlates of physical activity among adolescents. METHODS: Cross-sectional study nested within a cohort of 4,325 subjects from the city of Pelotas, Southern Brazil, aged 14-15 years in 2008. Physical activity was analyzed using three different approaches: (1) prevalence of any leisure-time physical activity; (2) prevalence of any active commuting to school; and (3) prevalence of engaging in at least 300 minutes per week of both (1) and (2) combined. Independent variables included sociodemographic, behavioral, social, and biological characteristics, and number of different leisure-time physical activites practiced. Statistical analyses were carried out using Poisson regression. RESULTS: The proportion of adolescents involved in any type of leisure-time physical activity was 75.6%, while 73.4% displayed some form of active commuting to school. Prevalence of total physical activity score (>; 300 min/week) was 48.2%, being greater among boys (62.6%) than among girls (34.5%). Furthermore, prevalence increased along with the number of physical activity modalities practiced (pOBJETIVO: Estimar la prevalencia de actividad fÃsica entre adolescentes y identificar los factores asociados. MÉTODOS: Estudio transversal anidado en una cohorte con 4.325 individuos de 14-15 años en Pelota, Sur de Brasil, en 2008. La actividad fÃsica fue analizada por medio de tres diferentes abordajes: 1) prevalencia de alguna actividad fÃsica de ocio; 2) prevalencia de algún traslado activo para la escuela; 3) prevalencia de compromiso de por lo menos 300 minutos por semana en la combinación de ambos (1 y 2). Variables independientes incluyeron caracterÃsticas demográficas, socioeconómicas, comportamentales, sociales y biológicas y número de actividades fÃsicas practicadas en el tiempo de ocio. Los análisis estadÃsticos fueron hechos por la regresión de Poisson. RESULTADOS: La proporción de adolescentes envueltos en alguna actividad fÃsica de ocio fue 75,6% mientras que 73,4% presentaron alguna forma de traslado activo para la escuela. La prevalencia de actividad fÃsica total (escore ³ 300 min/sem) fue 48,2% siendo mayor para los niños (62,6%) que para las niñas (34,5%). Además, la prevalencia aumentó de acuerdo con el número de actividades fÃsicas practicadas (pOBJETIVO: Estimar a prevalência de atividade fÃsica entre adolescentes e identificar fatores associados. MÉTODOS: Estudo transversal aninhado em uma coorte com 4.325 indivÃduos de 14-15 anos em Pelotas, RS, em 2008. Além disso, a prevalência aumentou de acordo com o número de atividades fÃsicas praticadas (p < 0,001). Os fatores associados à maior prática de atividade fÃsica (lazer + deslocamento) nos nÃveis recomendados foram: cor de pele não-branca, ter repetido de ano na escola e jogar videogame. As variáveis menor nÃvel socioeconômico, maior tempo de uso de computador e atividade fÃsica dos pais estiveram associadas ao desfecho apenas entre as meninas. CONCLUSÕES: Menos da metade dos adolescentes atingiu as recomendações para a prática de atividade fÃsica, e essa proporção tende a diminuir entre os sujeitos de maior nÃvel socioeconômico. Os fatores associados diferiram entre lazer e deslocamento. Engajamento em uma ampla variedade de atividades fÃsicas deve ser encorajado desde a infância
Infancy and childhood growth and physical activity in adolescence: prospective birth cohort study from Brazil.
BACKGROUND: The Developmental Origins of Health and Disease hypothesis suggests that intrauterine, infancy and early childhood variables play a key role at programming later health. However, little is known on the programming of behavioral variables, because most studies so far focused on chronic disease-related and human capital outcomes. The aim of the present study was to evaluate the effects of prenatal, infancy and childhood weight and length/height gains on objectively-measured physical activity (PA) in adolescence. METHODS: This is a prospective birth cohort study in Pelotas, Brazil, including 457 adolescents (mean age: 13.3 years) with weight and length/height data at birth, one, three and six months, one and four years of age. PA was measured using a GT1M Actigraph accelerometer, and expressed as (a) minutes per day spent on sedentary, light, moderate, vigorous and very-vigorous activities; (b) total counts per day. RESULTS: 61.3% of the adolescents accumulated 60+ minutes of moderate-to-vigorous PA per day. Weight and length/height trajectories in infancy and childhood were similar between those classified as active or inactive at 13.3 years. However, those classified as inactive were heavier and taller at all ages; differences were statistically significant only in terms of length at three, six and 12 months. CONCLUSIONS: Weight gain in infancy and childhood did not predict variability in adolescent PA, but those active in adolescence showed somewhat smaller average gains in length in infancy. Socioeconomic position and sedentary behavior in Brazilian adolescents: A life-course approach.
Socioeconomic position (SEP) is a potential correlate of sedentary behavior in adolescents. The aim of this study was to examine the associations between SEP and self-reported and objective measures of sedentary behavior in adolescents, using a life-course approach. Data from the 1993 Pelotas (Brazil) Birth Cohort Study were analyzed (N=5249). Cross-sectional and longitudinal associations between multiple SEP indicators (maternal education, family income, SEP composite, cumulative family income) at birth, 11, 15 and 18years, and five sedentary behavior outcomes (≥4h/day screen time; ≥4h/day TV; ≥2h/day computer; ≥2h/day video game; ≥12.7h/day objectively measured sedentary time) at 11, 15 and 18years, were examined. In cross-sectional analyses, higher SEP was positively associated with more screen time at ages 11 and 15years. There was a consistent and positive association between higher SEP with time spent using a computer, and with sedentary time assessed through accelerometry. SEP at birth had a positive and direct effect on screen, computer and total sedentary time at 18years. Participants in the highest cumulative income group had higher odds of high sedentary behavior in screen (OR: 2.40; 95% CI: 1.50-3.54), computer (OR: 7.35; 95% CI: 4.19-12.89) and total sedentary time (OR: 5.40; 95% CI: 3.53-10.35), respectively, compared with their counterparts with lower cumulative income. Our findings showed that SEP is an early determinant of sedentary behavior in adolescents
Evaluation of the Academia da Cidade program to promote physical activity in Recife, Pernambuco State, Brazil: perceptions of users and non-users
O objetivo foi descrever usuários e não-usuários do Programa Academia da Cidade de Recife, Pernambuco, Brasil. Foram selecionados quatro dos 19 pólos do programa. Para cada usuário sorteado, foi selecionado um não-usuário, que foi pareado ao usuário em termos de sexo, idade (±5 anos) e local de residência. A amostra foi composta por 277 usuários e 277 não-usuários. Entre os não-usuários, 22% referiram nunca ter ouvido falar do programa. Entre os que já ouviram falar do programa, 54,3% o conhecem por terem visto um pólo. Entre os usuários, o principal motivo para a participação no programa foi "melhorar a saúde"; o tempo médio de participação no programa foi de 27,5 meses (DP = 23,1) e 72,8% relataram grau elevado de satisfação. O percentual de pessoas com percepção de saúde regular ou ruim foi significativamente maior entre os não-usuários (45,4%) em comparação aos usuários (28,5%). Os usuários do Programa Academia da Cidade parecem estar satisfeitos. Estratégias de divulgação devem ser priorizadas para que o programa seja conhecido por um maior percentual de residentes de Recife.The aim of this study was to describe the profile of users and non-users of the Academia da Cidade Program to promote physical activity in Recife, Pernambuco State, Brazil. Four of the program's 19 units were sampled. Users (n=277) and non-users (n=277) were matched for gender, age (±5 years), and place of residence. Among non-users, 22% had never heard of the program. Among those who knew of the program, 54.3% learned about it by actually having seen one of the units. Among users, the main reason for participating in the program was to improve their health; mean participation time was 27.5 months (SD = 23.1); and 72.8% reported high satisfaction. The proportion of individuals with fair or poor self-rated health was significantly higher among non-users (45.4%) than users (28.5%). Users of the program appeared to be satisfied. Strategies to further publicize the program should be prioritized to make it known to a larger proportion of the city's population
Does objectively measured physical activity modify the association between early weight gain and fat mass in young adulthood?
BACKGROUND: Substantial evidence suggests that weight gain in early life is associated with increased adiposity and other metabolic disorders later in life. It is, however, unknown whether physical activity (PA) may modify these associations. We aimed to examine whether objectively measured PA at 30 years modified the associations between conditional weight gain in infancy (0-2 y) and childhood (2-4 y) with fat mass index (FMI) and visceral abdominal fat measured at age 30 years. METHODS: Prospective birth cohort study in Pelotas, Brazil, including 1874 participants with weight data at birth, two and four years of age, and measures of FMI, visceral abdominal fat and PA at a mean age of 30.2 years. At age 30, time spent (min/day) in moderate-to-vigorous physical activity (MVPA) was measured objectively using a wrist-worn accelerometer worn for four to seven consecutive days.. Multiple linear regression analyses was performed to assess the associations between conditional weight gain and outcome variables at 30 years, adjusting for covariates. We examined whether PA modified the association between conditional weight gain and the outcomes of interest by introducing an interaction term (conditional weight gain × PA) in the models. RESULTS: Conditional weight gain in infancy and childhood were both positively associated with later FMI (infancy weight gain: β = 0.68, 95% CI: 0.48, 0.88; P < 0.001; childhood weight gain: β = 0.91, 95% CI: 0.70, 1.11; P < 0.001). A formal test for interaction suggested that MVPA at 30 years of age modified the association between childhood relative weight gain and later FMI (β = -0.006, 95% CI: -0.011, -0.001; P = 0.029), suggesting stronger associations between weight gain and FMI in those with lower levels of MVPA. Conditional weight gain in childhood was also positively associated with visceral abdominal fat (β = 0.24, 95% CI: 0.15, 0424, P < 0.001). There was no evidence for a modification of the latter association after adjustment for physical activity. CONCLUSION: Conditional weight gain between 2 and 4 years of age is associated with increased FMI at age 30 years.
